Abstract: Introdução: A empatia na relação médico-doente é uma ferramenta essencial na prática clínica, havendo evidência da sua associação a melhores consequências em saúde. Contudo, vários estudos têm demonstrado que a empatia vai diminuindo ao longo da formação pré-graduada em Medicina. Tal fenómeno implica a necessidade de perceber as suas causas e determinantes para que seja possível desenhar programas de Educação Médica Contínua e Específica. Objetivos: Perceber as determinantes da diminuição dos valores de empatia ao longo do curso de medicina para que seja possível corrigir tal trajetória. Material e Métodos: Estudo observacional e transversal por questionário de autopreenchimento em anonimato, sigilo e confidencialidade, numa amostra representativa do Universo dos estudantes do Mestrado Integrado em Medicina da Faculdade de Medicina da Universidade de Coimbra no ano letivo 2017/2018, recorrendo a aplicação informática. Para a preparação do questionário foi realizado um focus group inicial e depois um estudo piloto com 30 alunos para melhorar o questionário.Resultados: Resultados: Através da Análise Fatorial Confirmatória, as quarenta afirmações acerca dos determinantes da queda da empatia percebidos no focus group foram identificados e classificados em doze fatores. Após aplicação do questionário, a análise pelo coeficiente alfa de Cronbach mostrou boa consistência interna (0,895). “Mecanização da prática” é o fator com maior concordância, seguido de “O exemplo dos tutores”, “Aprendizagem tutelada” e “Necessidade de prática”. Não se verificam diferenças significativas entre as respostas dos sexos masculino e feminino. Entre grupos pré-clínico e clínico, verificam-se diferenças significativas para o fator “Necessidade de prática” (p=0,004), “Aprendizagem dirigida” (p=0,035) e “Aprendizagem tutelada” (p=0,029).Discussão e Conclusão: O estudo encontrou doze fatores determinantes para a queda da empatia médica nos estudantes de medicina, sendo eles: “Ensino teórico”, “Necessidade de prática”, “Aprendizagem dirigida”, “Aprendizagem tutelada”, “Comunicação com o doente”, “Impacto negativo da vida social extra-curricular”, “Despersonalização pela carga curricular”, “Mecanização da prática”, “Modelo e sua prática conflituantes”, “Preocupações com a prática”, “O exemplo dos tutores” e “A importância da socialização”, comprovando-se a necessidade de implementar novos programas na educação médica, com o objetivo de combater este declínio da empatia por parte dos estudantes. Sugere-se que se realizem mais estudos nesta área.
Background: Empathy is an essential tool in clinical practice, and there is evidence of its association with improved clinical outcomes. However, several studies have shown that empathy decreases throughout the medical course. This decline of empathy supports the need to enhance empathic ability through Continuing and Specific Medical Education programs.Objectives: To understand the determinants of the decrease of the empathy values ​​so that it is possible to reduce their predisposing factors.Materials and Methods: Observational and cross-sectional study by self-filling form in anonymity and confidentiality, in a representative sample in a universe of students from FMUC - Integrated Master 's Degree in Medicine, during the 2017/2018 academic year, using a computer application. An initial focus group was carried out for the preparation of the form and then a pilot study with thirty students to improve the questionnaire.Results: Through the Confirmatory Factor Analysis, the forty statements about the determinants of the decline of empathy were identified and classified into twelve factors. After application of the questionnaire, Cronbach's alpha coefficient analysis showed good internal consistency (0.895). "Mechanization of practice" was the best classified factor, followed by the factor "The example of the tutors", "Tutored learning" and "Need for practice". There are no statistically significant differences between male and female responses. In the preclinical and clinical groups, the results indicate that there are statistically significant differences for the "Need for practice" factor (p <0.01), "Targeted learning" and "tutored learning" factors (p <0.05). There were no statistically significant differences for the remaining factors.Discussion and Conclusion: The study found twelve determinant factors for the decline of medical empathy in medical students: “Theory teaching”, “Need for practice”, “Targeted learning”, “Tutored learning”, “Communication with the patient”, “Negative impact in social life”, “Depersonalization by curricular load”, “Mechanization of practice”, “Conflicting model and practice”, “Concerns about practice”, “The example of tutors” and “The meaning of socialization”, proving the need to implement new programs in medical education, in order to combat this decline in student empathy.It is suggested that more studies be done in this area.
