Título: Impacto da esperança e da espiritualidade no padrão emocional dos idosos

Resumo: Enquadramento: O aumento da longevidade da população mundial e nacional nas próximas décadas promoverá um aumento considerável da população idosa com grande impacto na gestão da assistência em saúde por quanto o envelhecimento provoca alterações em momentos e intensidades diferentes, e dependem de características genéticas, ambientais e sociais. Este processo tende a ser doloroso para muitos idosos, que frequentemente se deparam com o isolamento, a falta de apoio social, a dificuldade em lidar com o próprio processo de envelhecimento, com a morte do cônjuge, o abandono familiar, o que também pode desencadear doenças físicas/psíquicas e/ou índices elevados de sofrimento emocional, depressão, ansiedade e revolta. Neste âmbito, e tendo em conta que a esperança e a espiritualidade integram a componente cognitiva comportamental da dimensão socio-espiritual da pessoa, é fundamental avaliar se estas dimensões têm impacto nas vivências dos idosos. Objetivos: Avaliar o padrão emocional da pessoa idosa. Determinar se as variáveis sociodemográficas, clínicas, a esperança e a espiritualidade influenciam o padrão emocional da pessoa idosa. Métodos: O estudo descritivo-correlacional e transversal foi realizado com 161 idosos (63 institucionalizados e 98 a residir no domicílio/com família), habitantes na zona centro de Portugal continental, com idades compreendidas entre os 65 e os 96 anos de idade (x=77,27 anos). O instrumento de recolha de dados integrou um questionário de caracterização sociodemográfica e clínica, a escala da Esperança (Herth Hope Index), a escala de Bem-Estar Espiritual (FACIT-Sp-12) e os Termómetros Emocionais. Resultados: Os resultados para a amostra global mostram que: - Os idosos apresentam bons índices médios de esperança; - A esperança é maior nas mulheres e menos significativa nos idosos mais velhos e com menos habilitações literárias; - O bem-estar espiritual é maior nos idosos institucionalizados, nos que não têm filhos e nos que têm doença crónica; - Os idosos apresentam um bom padrão emocional (scores <5), com valores superiores a 75,2%; - O padrão emocional é mais positivo nos homens, evidenciando as mulheres maior índice de depressão. Os idosos que vivem em meio urbano apresentam maior sofrimento emocional (p=0,018), mais ansiedade (p=0,048), depressão e revolta, mas menos necessidade de ajuda. O aumento da idade (p=0,040) e viver sozinho (p=0,040) são fatores que aumentam a depressão nos idosos. Verificou-se uma correlação inversa e significativa entre a esperança, o bem-estar espiritual e todas as dimensões do padrão emocional, inferindo-se que bons índices de esperança e espiritualidade se associam a vivências emocionais mais positivas na pessoa idosa. Conclusão: A esperança e espiritualidade são inerentes ao ser humano e acompanham-no ao longo do seu ciclo vital, sendo preponderantes em fases de transição da vida como na terceira idade, verificando-se que são um recurso efetivo para melhorar significativamente o padrão emocional da pessoa idosa. Impõe-se por isso incluir medidas promotoras da esperança e do bem-estar espiritual nos planos assistenciais para os idosos.
Abstract Context: Increasing the longevity of the world and national population in the coming decades will promote a considerable increase in the elderly population with a major impact on the management of health care because aging causes changes at different times and intensities and depends on genetic, environmental and social rights. This process tends to be painful for many older people, who often face isolation, lack of social support, difficulty in coping with their own aging process, death of the spouse, family neglect, which can also trigger physical / psychic illnesses and / or high rates of emotional distress, depression, anxiety and revolt. In this context, and considering that hope and spirituality integrate the cognitive behavioral component of the socio-spiritual dimension of the person, it is fundamental to evaluate if these dimensions impact on the experiences of the elderly. Aims: To evaluate the emotional pattern of the elderly person. To determine if sociodemographic, clinical, hope and spirituality variables influence the emotional pattern of the elderly person. Methodology: The descriptive-correlational and cross-sectional study was carried out with 161 elderly people (63 institutionalized and living in the home / with family), inhabitants of central Portugal, aged between 65 and 96 years (x = 77.27 years). The data collection instrument integrated a sociodemographic and clinical characterization questionnaire, the Herth Hope Index, the Spiritual Well-being scale (FACIT-Sp-12) and the Emotional Thermometers. Results: The results for the global sample reveal that: - The elderly have good average rates of hope; - The hope is greater in women and less significant in older people with less literacy; - Spiritual well-being is greater in the institutionalized elderly, in those who do not have children and in those who have chronic illness; - The elderly have a good emotional pattern (scores <5), with values higher than 75.2%; - The emotional pattern is more positive in men, showing women a higher rate of depression. Older people living in urban areas have greater emotional distress (p = 0.018), more anxiety (p = 0.048), depression and revolt, but less need for help. Increasing age (p = 0.040) and living alone (p = 0.040) are factors that increase depression in the elderly. There was an inverse and significant correlation between hope, spiritual well-being and all dimensions of the emotional pattern, and it was inferred that good indices of hope and spirituality are associated with more positive emotional experiences in the elderly person. Conclusion: Hope and spirituality are inherent to the human being and accompany him throughout his life cycle, being preponderant in the transition phases of life as in the third age, being verified that they are an effective resource to significantly improve the emotional pattern of the elderly. It is therefore necessary to include measures that promote hope and spiritual well-being in the care plans for the elderly.
