Abstract
Bakgrunn Mykofenolatmofetil (MMF, CellCept®) er et immunosuppressivt prodrug som i kroppen raskt metaboliseres til den aktive metabolitten mykofenolsyre (MPA). MPA brukes profylaktisk mot avstøtning etter organtransplantasjon (nyre, lever, hjerte), som oftest i kombinasjon med en kalsinevrinhemmer og/eller kortikosteroider. Det er store individuelle forskjeller i plasmakonsentrasjon etter oral administrasjon av MMF. Lav plasmakonsentrasjon av MPA øker risikoen for avstøtning av det transplanterte organet, mens for høye konsentrasjoner kan gi en rekke uønskede bivirkninger. En sentral faktor som bidrar til de store forskjellene i farmakokinetikken til MPA mellom individer er enterohepatisk resirkulasjon (EHC) av MPA. Mål Målet med denne oppgaven var å utvikle en ikke-parametrisk farmakokinetisk populasjonsmodell for MMF, med fokus på EHC av MPA, hos levertransplanterte som i tillegg blir behandlet med takrolimus og steroider. Metode Det ble utført detaljerte 12-timers farmakokinetikkundersøkelser hos 16 levertransplanterte pasienter (inntil 3 uker posttransplantasjon). Dette resulterte i 56 tids-/konsentrasjonskurver, med til sammen 723 konsentrasjonsmålinger, som dannet datagrunnlaget for populasjonsmodelleringen. Modelleringen ble utført i Pmetrics versjon 1.3.1. Det ble utviklet en passende strukturell modell som kunne beskrive EHC hos testsubjektene. Resultater En 5-kompartmentmodell med 1. ordens kinetikk og lagtid i absorpsjonsfasen gav en god beskrivelse av farmakokinetikken til MFF på individnivå. EHC ble modellert ved hjelp av et eget galleblærekompartment, der sekresjon av MPA tilbake til gastrointestinaltrakten ble kontrollert av parameterestimert tid. Reabsorpsjon fra tykktarmkompartment klarte å beskrive sekundære topper i MPA konsentrasjon-/tidskurver. De individuelle predikerte versus observerte plottene med og uten kovariater hadde henholdsvis R2 = 0.946 og R2 = 0.967. Begge hadde i tillegg lave verdier og bias (-0.0694 og -0.138) og imprecision (2.32 og 1.15 ). Imidlertid var modellens prediksjonsevne svak på populasjonsnivå, og modellen vil ha utfordringer med å predikere nye pasienter. Modellen med kovariater gjorde det best med R2 = 0.329, bias = 0.746 og imprecision = 15.9. Modellen uten kovariater hadde R2 = 0.228, bias = 1.26 og imprecision = 22.5. Konklusjon Populasjonsmodellen klarte å beskrive EHC av MPA hos levertransplanterte på individnivå. Imidlertid vil modellen ha store utfordringer ved prediksjon av nye pasienter på populasjonsnivå. Rike kinetikkdata fra flere pasienter bør inkluderes og fokus på kovariater som kan predikere individuelle EHC bør testes ut i den videre modellutviklingen.
Background: Mycophenolate mofetil (MMF, CellCept®) is an immunosuppressive prodrug, which rapidly metabolize to the active metabolite mycophenolic acid (MPA) in the human body. MPA is used prophylactically against graft rejection after organ transplantation (kidney, liver, heart), usually in combination with a calcineurin inhibitor and/or corticosteroids. There are large individual differences in plasma concentration after oral administration of MMF. A low plasma concentration of MPA increases the risk of graft rejection, whereas high concentrations may cause a variety of unwanted side effects. A key factor contributing to the large differences in the pharmacokinetics of MPA between individuals is enterohepatic recirculation (EHC) of MPA. Goal: The aim of this thesis was to develop a non-parametric pharmacokinetic population model for MMF, with special emphasis on EHC of MPA, in liver transplant recipients co-administered with tacrolimus and steroids. Methods: Data rich pharmacokinetic investigations were performed at repeated times, up to three weeks after transplantation, in 16 liver transplant recipients. Fifty-six 12-hour time/concentration curves, in total 723 MPA plasma concentrations, formed the basic data material for the model development. The modeling was performed in Pmetrics version 1.3.1. An appropriate structural model that described EHC was developed and patient covariates tested for inclusion in the model. Results: A 5-compartment model with 1. order kinetics and lag time in the absorption phase fitted the data well at the individual level. EHC was modeled applying a separate “gallbladder compartment”, where secretion of MPA back to the gastrointestinal tract (“gall bladder emptying”) was governed by a time parameter. Model reabsorption from the “intestine compartment” was able to describe secondary peaks in the MPA plasma concentration versus time curves. The coefficient of determination for the individual predicted versus observed plots, with and without covariates, was R2 = 0.967 and R2 = 0.946, respectively. Additionally, bias (-0.138 and -0.0694) and imprecision (1.15 and 2.32) were low for both models. However, the model has poor predictive power at the population level, and will not perform well in predicting the pharmacokinetics of new patients. The model estimates were (without covariates) R2 = 0.228, bias = 1.26 and imprecision = 22.5. The model with covariates performed slightly better with R2 = 0.329, bias = 0.746 and imprecision = 15.9. Conclusion: The population model described EHC of MPA in liver transplant recipients at the individual level. However, the model has poor predictive power at the population level. Rich kinetic data from a greater pool of patients should be included and a special aim to detect covariates relevant to timing and degree of individual EHC should be the future focus of the further development of the model.
