Resumen/Abstract
RESUMEN: La introducción del uso de la endoprótesis se ha postulado como una alternativa eficaz a la cirugía urgente en el tratamiento de la oclusión intestinal por cáncer colorrectal. El objetivo principal de nuestro estudio es comparar ambos métodos de tratamiento para valorar las posibles diferencias de morbimortalidad, necesidad de estomas y resultados oncológicos a largo plazo.
Se trata de un estudio retrospectivo, controlado y no aleatorizado que se hizo en el servicio de cirugía general del HUMV (Hospital Universitario Marqués de Valdecilla). El estudio contó con una muestra de 129 pacientes con obstrucción intestinal secundaria a cáncer colorrectal izquierdo, tratados con endoprótesis como puente a una cirugía programada (n=49) y mediante cirugía urgente (n=80). Nuestras principales variables fueron las complicaciones postoperatorias, el tipo de intervención quirúrgica, la necesidad de estoma y los resultados oncológicos a largo plazo. Estas variables fueron comparadas sobre las bases de un análisis por “intención de tratar”.
Resultados: Obtuvimos diferencias significativas en las complicaciones postoperatorias (53,8% en el grupo de cirugía urgente frente a un 24,5% en el grupo de las endoprótesis) y en la necesidad de estoma (57.5% en el grupo de cirugía urgente frente a 24,5% del grupo de las endoprótesis). El éxito técnico fue del 95,9%, y el éxito clínico fue del 83%. No encontramos diferencias en cuanto a los resultados oncológicos a largo plazo.
Conclusión: Podemos decir que, el uso de la endoprótesis en el tratamiento de la obstrucción intestinal aguda por cáncer colorrectal izquierdo es una técnica segura y eficaz que reduce las complicaciones postoperatorias, disminuye la necesidad de colocación de un estoma y con unos resultados oncológicos a largo plazo no significativamente diferentes a los obtenidos en el grupo de la cirugía urgente.
ABSTRACT: The introduction of the use of the endoprosthesis has been postulated as an effective alternative to emergency surgery in the treatment of intestinal occlusion due to colorectal cancer. The main objective of our study is to compare both methods of treatment to assess the possible differences in morbidity and mortality, the need for stomata and long-term oncological results.
This is a retrospective, controlled and non-randomized study that was done in the general surgery service of HUMV (University Hospital Marqués de Valdecilla). The study included a sample of 129 patients with intestinal obstruction secondary to left CRC, treated with stenting as a bridge to a scheduled surgery (n = 49) and through urgent surgery (n = 80). Our main variables were postoperative complications, the type of surgical intervention, the need for a stoma and the long-term oncological results. These variables were compared on the basis of an "intention to treat" analysis.
Results: We obtained significant differences in postoperative complications (53.8% in the urgent surgery group versus 24.5% in the group of endoprostheses) and in the need for stoma (57.5% in the emergency surgery group versus to 24.5% of the group of endoprostheses). The technical success was 95.9%, and the clinical success was 83%. We did not find differences in terms of long-term oncological results.
Conclusion: We can say that the use of the stent in the treatment of acute intestinal obstruction due to left colorectal cancer is a safe and effective technique that reduces postoperative complications, decreases the need for stoma placement and with long-term oncological results. term not significantly different from those obtained in the urgent surgery group.
