Effectiveness of the 3rd and successive lines of systemic treatment in patients with metastatic colorectal cancer

Resumen/Abstract
RESUMEN:
Introducción. El regorafenib, el TAS-102 y la terapia anti-EGFR son las principales opciones terapéuticas aprobadas para los pacientes con cáncer colorrectal metastásico (CCRm) que hayan progresado después de recibir dos líneas de terapia sistémica. A pesar de ello, el beneficio clínico y el incremento de la supervivencia global obtenidos con estos tratamientos, según los datos de los ensayos clínicos, son escasos, aproximadamente 4-6 meses. En este estudio se pretende valorar la eficacia y toxicidad de estos tratamientos en la práctica real con pacientes no seleccionados. Métodos. Estudio retrospectivo que analiza pacientes con diagnóstico de CCRm tratados en el Hospital Universitario Marqués de Valdecilla, que han recibido terapia en 3ª y/o sucesivas líneas con los tratamientos aprobados en ésta entre abril de 2011 y marzo de 2019.Resultados Un total de 46 pacientes han sido incluidos y analizados en este estudio. Los tratamientos evaluados en 3ªlínea fueron regorafenib (50%), terapia anti-EGFR (39,1%) y TAS-102 (10,9%). Los efectos adversos más comunes fueron síndrome mano-pie (47,8%) y naúseas (39,1%) en los enfermos tratados con regorafenib, anemia (66,7%) y alteraciones hepáticas (44,4%) con terapia anti-EGFR y anemia (80%) y alteraciones hepáticas (60%) con TAS-102. Valorando la respuesta objetiva, un 8,7% de los enfermos tratados con regorafenib alcanzaron respuesta parcial (RP) y un 13% enfermedad estable (EE), en el caso de tratados con terapia anti-EGFR un 5,6% en ambos casos (RP/EE) y en los que recibieron TAS-102 un 20% alcanzaron cada una de las respuestas mencionadas (RP/EE). El resto de pacientes sufrieron progresión. La mediana de supervivencia libre de progresión (SLP) en la población global fue de 4 meses (IC 95%: 2,6-5,4), en el subgrupo de regorafenib fue de 4 meses (IC 95%:2,5-6), en el de anti-EGFR 4 meses (IC 95%:0,9-5,1) y en el de TAS -102 3 meses (IC 95%:0,9-5,2). La mediana de supervivencia global (SG) para nuestra población fue de 5 meses (IC 95%:1-5), 7 meses (IC 95%:3,3-10,7) en los pacientes con regorafenib, 3 meses con anti-EGFR (IC 95%:1-5,1) y 3 meses con TAS-102 (IC 95%:0,9-5,2). Conclusiones. Las características de los pacientes, las toxicidades documentadas y la eficacia de los tratamientos fueron similares y comparables con las observadas en los ensayos de referencia.
ABSTRACT:
Introduction Regorafenib, TAS-102 and anti-EGFR therapy are the main therapeutic options currently approved for patients with metastatic colorectal cancer (mCRC) in whom progression of the disease has occurred after receiving two lines of systemic therapy. In spite of this, the clinical benefit and the increase of the overall survival obtained with these treatments according to the data of the clinical trials are scarce, approximately 4-6 months. This study aims to assess the efficacy and toxicity of these treatments in current practice with non-selected patients. Methods. Retrospective study that analyzes patients diagnosed with mCRC in the HUMV who have received therapy in the 3rd and / or successive lines with the treatments approved on this between April 2011 and March 2019. Findings A total of 46 patients were included and analyzed in this study with the data obtained. The treatments evaluated in 3rd line were regorafenib (50%), anti-EGFR therapy (39.1%) and TAS-102 (10.9%). The most common adverse effects were hand-foot syndrome (47.8%) and nausea (39.1%) in patients treated with regorafenib, anemia (66.7%) and liver disorders (44.4%) with anti-EGFR therapy, and anemia (80%) and hepatic alterations (60%) with TAS-102. Assessing the objective response, 8.7% of patients treated with regorafenib achieved a partial response (PR) and 13.04% stable disease (SD), in the case of patients treated with anti-EGFR therapy 5.6% in both cases (PR/SD), and in those who received TAS- 102 20% reached each of the responses mentioned (PR/SD). The rest of the patients suffered progression. The median progression-free survival (PFS) for the overall population was 4 months (95% CI: 2.6-5.4), in the subgroup of regorafenib was 4 months (95% CI: 2.5- 6), in the case of anti-EGFR 4 months (95% CI: 0.9- 5.1) and in that of TAS -102 3 months (95% CI: 0.9-5.2). The mean overall survival (OS) in our population was 5 months (95% CI: 1.01-3.02), 7 months (95% CI: 3.3- 10.7) for patients with regorafenib, 3 months (95% CI: 1-5,1) with anti-EGFR and 3 months (95% CI: 0.9-5.2) with TAS-102. Interpretation. The characteristics of the patients, the toxicities documented and the efficacy of the treatments were similar and comparable with those previously reported in the pivotal clinical trials.
