Amyotrophic lateral sclerosis (ALS) is a largely sporadic progressive neurodegenerative disease affecting upper and lower motor neurons (MNs) and specific etiology that is not completely understood. Up to now, there is no clinically relevant treatment available for ALS. The novel therapeutic strategies should aim at neuroprotection, stimulation of intrinsic neurogenesis and replacement of affected neural cells. These could be achieved by application of powerful modern therapeutic methods such as gene therapy, transplantation of different types of engineered and non-engineered stem cells. Cell-based therapies could facilitate either restoration/substitution or the preservation of MNs. Special attention has been paid to treatment with autologous stem cells, as the most promising approach to the treatment of ALS patients. This review summarises the progress made in translating to the patients' bedside the results achieved from in vivo preclinical studies.
