Resumo

The mild gestational hyperglycemia (MGH) and gestational diabetes mellitus (GDM), referred as gestational hyperglycemia in this study, have important consequences for the mother, fetus and newborn. The association between gestational hyperglycemia and urinary incontinence is scarce in the literature. Our academic research group "Diabetes and Pregnancy - Clinical and Experimental", from Botucatu Medical School, UNESP, has studied the translational aspects of gestational hyperglycemia related to urinary incontinence in females and in animal models. The results help to elucidate the pathophysiological mechanisms that lead to female pelvic floor disorders in women with GDM and HGL, but there is need for further research to expand the understanding of the phenomena that result in better care for patients. Whereas monocyte chemotactic protein-3 (CCL7) is an important factor for the recovery mechanism of urinary incontinence, our hypothesis was that patients with GDM or HGL present changes in the CCL7 levels and this would retard/prevent the migration of mesenchymal stem cells to the site of injury caused by IU, causing consequences in the postpartum period. Thus, the purpose of this study was to determine the CCL7 levels profile in hyperglycemic pregnant women with urinary incontinence since the beginning of pregnancy up to the first year postpartum and to examine the correlation between the progression of UI and serum levels of CCL7. This identification may indicate CCL7 as a possible biomarker of recovery UI in hyperglycemic women during pregnancy and in the postpartum period and subsequently establish effective therapeutic intervention with consistent pathophysiological basis. Statistical analysis and the choice by comparing tests between variables and groups were executed respecting the presuppositions determined by the results, characteristics and behavior of the variables.
A hiperglicemia gestacional leve (HGL) e o Diabetes mellitus gestacional (DMG), denominados como hiperglicemia gestacional no estudo, têm consequências importantes para a mãe, o feto e o recém-nascido. A associação entre hiperglicemia gestacional e incontinência urinária é escassa na literatura. Nosso grupo de pesquisa acadêmico "Diabete e Gravidez - Clínico e Experimental" da Faculdade de Medicina de Botucatu-Unesp tem estudado os aspectos translacionais da hiperglicemia gestacional relacionada à incontinência urinária no sexo feminino e também em modelos animais. Os resultados ajudam a elucidar os mecanismos fisiopatológicos que conduzem aos distúrbios do assoalho pélvico feminino em mulheres com DMG e HGL, mas há necessidade de mais pesquisas para ampliar a compreensão dos fenômenos que resultarão no melhor atendimento às pacientes. Considerando que a proteína quimiotática de monócitos-3 (CCL7) é um importante fator para o mecanismo de recuperação da incontinência urinária, nossa hipótese foi que as pacientes portadoras de DMG ou de HGL apresentariam alteração nos níveis de CCL7 e isto retardaria/impediria a migração de células tronco mesenquimais para o local de lesão causada por IU, causando consequências no período pós-parto. Assim, a proposta deste estudo foi estudar os níveis de CCL7 em gestantes hiperglicêmicas com incontinência urinária desde o início da gestação até o primeiro ano pós-parto, além de examinar as correlações entre a progressão da incontinência urinária e os níveis séricos de CCL7. Esta identificação poderia indicar o CCL7 como possível biomarcador de recuperação da IU nas mulheres hiperglicêmicas no período gestacional e pós-parto e posteriomente estabelecer intervenção terapêutica eficaz com base fisiopatológica consistente. A análise estatística, bem como a escolha pelos testes de comparação entre as variáveis e grupos foram executadas respeitando os pressupostos determinados pelos resultados, características e comportamento das variáveis.
