Percepção de mães de bebês internados em UTI neonatal: Influência de variáveis maternas, contextuais, apoio social e enfrentamento

Resumo

A unidade de terapia intensiva neonatal (UTIN), embora seja um ambiente de cuidados especiais para garantir a sobrevivência de recém-nascidos, tende a suscitar, nas mães e familiares percepções e sentimentos ambíguos sobre os eventos relacionados a internação e, ainda, alterar a saúde emocional dos envolvidos. Esses aspectos podem ser influenciados pelo tempo de internação do bebê. O presente projeto foi dividido em duas etapas. Na primeira Etapa 1 pretendeu-se elaborar e adequar instrumentos que possibilitassem a investigação da percepção e de sentimentos que mães de bebês internados em UTIN têm a respeito desta condição. O resultado foi a elaboração dos instrumentos: Protocolo para avaliação da percepção materna sobre a condição do bebê internado em UTIN, com 24 itens e, o Protocolo de avaliação de sentimentos, com três itens de múltipla escolha, para identificação da frequência e justificativa dos sentimentos emergidos no momento da notícia, durante e após a hospitalização. A Etapa 2 é composta por cinco estudos, com o objetivo de avaliar os sentimentos e a percepção materna sobre a internação, a saúde emocional materna, o apoio social percebido e as estratégias de enfrentamentos, relacionando-os com o tempo de internação. Os dados de todos eles foram obtidos de uma amostra de 50 mães de bebês internados em UTIN da Maternidade Santa Isabel, da cidade de Bauru/SP, há pelo menos três dias. Utilizou-se para a avaliação da saúde emocional materna a Escala de Estresse Percebido, o Inventário de Ansiedade Traço-Estado e o Inventário de Depressão de Beck. Para a avaliação do enfrentamento foi aplicada a Escala Modos de Enfrentamento de Problemas e o apoio social foi avaliado pela Escala de Apoio Social (EAS). No Estudo 1 o objetivo foi descrever a saúde emocional de mães de bebês internados, comparando-a e relacionando-a com o tempo de internação em UTIN. Os resultados apontaram para 12% de mães com indicadores clínicos para depressão, 38% para estresse alto e moderado, 64% de mães com indicadores clínicos para ansiedade estado e 54% para ansiedade traço. Delas 56% apresentaram dois ou mais indicadores clínicos. Houve correlação positiva entre ansiedade estado e o tempo de internação na UTIN. No Estudo 2 o objetivo foi descrever a rede de apoio social percebida pelas mães de bebê em UTIN e relacioná-la com o tempo de internação. Observou-se que as mães percebem mais o apoio de familiares, principalmente nas dimensões material, afetivo e informacional. Não houve correlação entre as dimensões de apoio social avaliadas e o tempo de internação. O Estudo 3 teve como objetivo descrever as estratégias de enfrentamento maternas, comparando-as considerando o tempo de internação e número de indicadores clínicos. Os resultados mostraram que das estratégias utilizadas as mais frequentes foram as focalizadas no problema. Não foram observadas correlações com tempo de internação e as estratégias utilizadas. A depressão foi positivamente associada com estratégias focalizadas na emoção e negativamente associada com estratégias focalizadas no problema. O Estudo 4 teve como objetivos descrever os sentimentos das mães em diferentes momentos da internação e compará-los considerando ausência e presença de indicadores clínicos. Os resultados apontaram que mães relataram mais sentimentos negativos no momento da notícia da internação e menos nos momentos da visita. A maioria relatou sentimentos positivos com relação à expectativa da alta hospitalar. Mães que relataram sentimentos negativos por ocasião da notícia também apresentaram mais indicadores durante as visitas, mesmo relatando sentimentos positivos. O Estudo 5 teve como objetivo descrever a percepção materna sobre a internação do bebê em UTIN e correlacioná-la com a saúde emocional materna. Os resultados obtidos mostraram que a percepção materna se divide entre positiva e negativa em quatro das cinco categorias avaliadas. Somente a percepção de apoio social foi percebida como negativa e associou-se negativamente com ansiedade estado. Os dados obtidos sugerem a importância de identificar como as mães percebem, sentem e os desdobramentos da vivência da internação de seus bebês para a implementação de intervenções no espaço hospitalar.
The neonatal intensive care unit (NICU), although it is a special care environment to ensure the survival of newborns, tends to raise in mothers and relatives ambiguous perceptions and feelings about hospitalization events and change the emotional health of those involved. These aspects can be influenced by the length of time the baby is hospitalized. This project was divided into two stages. In the first stage 1, it was intended to elaborate and adapt instruments that would enable the investigation of the perception and feelings that mothers of infants admitted to NICU have regarding this condition. The result was the elaboration of the instruments: Protocol for the evaluation of maternal perception on the condition of the baby hospitalized in NICU, with 24 items and the Protocol of feelings evaluation, with three multiple-choice items, to identify the frequency and justification of feelings emerged at the time of the news, during and after hospitalization. The Step 2 is composed of five studies, with the objective of evaluating maternal feelings and perceptions about hospitalization, maternal emotional health, perceived social support and coping strategies, relating them to length of stay. Data from all of them were obtained from a sample of 50 mothers of infants admitted to a NICU at Maternidade Santa Isabel, in the city of Bauru / SP, for at least three days. The Perceived Stress Scale, the State-Trait Anxiety Inventory, and the Beck Depression Inventory were used to assess maternal emotional health. For the evaluation of the coping was applied the Scale Modes of Confronting Problems and the social support was evaluated by the Scale of Social Support (EAS). In Study 1 the objective was to describe the emotional health of mothers of hospitalized babies, comparing it and relating it to the length of stay in NICU. The results pointed to 12% of mothers with clinical indicators for depression, 38% for high and moderate stress, 64% of mothers with clinical indicators for state anxiety and 54% for trait anxiety. Of these, 56% had two or more clinical indicators. There was a positive correlation between state anxiety and length of hospital stay in the NICU.In Study 2 the objective was to describe the network of social support perceived by baby mothers in NICU and to relate it to the length of hospital stay. It was observed that mothers perceive more the support of relatives, especially in the material, affective and informational dimensions. There was no correlation between the dimensions of social support assessed and the length of hospital stay. Study 3 aimed to describe the strategies of maternal coping, comparing them considering the time of hospitalization and number of clinical indicators. The results showed that the most frequent strategies were those focused on the problem. No correlations were observed with length of hospital stay and the strategies used. Depression was positively associated with strategies focused on emotion and negatively associated with strategies focused on the problem. Study 4 had as objectives to describe the feelings of the mothers at different moments of the hospitalization and to compare them considering absence and presence of clinical indicators. The results showed that mothers reported more negative feelings at the time of the hospitalization notice and less at the time of the visit. Most reported positive feelings regarding the expectation of hospital discharge. Mothers who reported negative feelings at the time of the news also showed more indicators during the visits, even reporting positive feelings. Study 5 aimed to describe the maternal perception about the infant's hospitalization in NICU and to correlate it with maternal emotional health. The results showed that maternal perception is divided between positive and negative in four of the five categories evaluated. Only the perception of social support was perceived as negative and was associated negatively with state anxiety. The data obtained suggest the importance of identifying how mothers perceive, feel and unfold the experience of the hospitalization of their babies for the implementation of interventions in the hospital space.
