Estudo dos mecanismos envolvidos no desenvolvimento de resistência à insulina em ratos com lesão periapical

Resumo

Nos últimos anos, a relação entre infecções orais e desordens sistêmicas tem se consolidado como área de grande interesse na comunidade cientifica médica e odontológica. A lesão periapical (LP) é caracterizada como uma inflamação oral e está associada ao aumento da quantidade de citocinas pró-inflamatórias que possivelmente induzem resistência insulínica (RI). A RI é definida como a incapacidade dos tecidos periféricos em responder adequadamente às concentrações fisiológicas deste hormônio, no entanto, os mecanismos que causam RI não são totalmente compreendidos. Estudos anteriores do nosso laboratório observaram que a LP promove aumento das concentrações plasmáticas de TNF-α, prejuízos na transdução do sinal insulínico e redução do conteúdo de GLUT4 na membrana plasmática em tecido muscular esquelético, indicando uma relação entre LP e RI. Tais achados evidenciam a necessidade de realizar mais estudos para verificar os mecanismos envolvidos nestas alterações. O presente estudo, conduzido em ratos com LP, teve como objetivos: 1) calcular o índice HOMA-IR a partir dos concentrações plasmáticas de glicose e insulina 2) avaliar conteúdo total das proteínas inflamatórias (JNK, IKKα/ e TNF-) no músculo gastrocnêmio (MG) e o grau de fosforilação de JNK e IKKα/ no mesmo tecido; 3) verificar a presença de macrófagos infiltrados por meio da detecção da proteína F4/80 em MG; 4) quantificar a expressão de fatores de transcrição envolvidos com a diferenciação de linfócitos no baço (T-bet, GATA3 e FOXP3); 5) analisar as concentrações plasmáticas de LPS, HSP70, INF, IL-4, TGF- . Para tanto, foram utilizados 45 ratos Wistar (2 meses de idade) distribuídos em três grupos: a) ratos do grupo controle, sem a LP (CN); b) ratos com LP induzida em primeiro molar superior direito (apenas uma lesão) (1LP); c) ratos com LP induzida em primeiros e segundos molares superiores e inferiores do lado direito (total de quatro lesões) (4LP). A LP foi induzida empregando-se broca em aço carbono dotada de esfera na extremidade com 0,1 mm. A análise estatística foi realizada por análise de variância (ANOVA), seguida pelo teste de Tukey. O nível de significância adotado foi de 5% (alfa=5%). Os resultados demonstraram uma redução na sensibilidade à insulina e um aumento do conteúdo de macrófagos ao redor das fibras musculares nos grupos LP. O grau de fosforilação de JNK e IKKα/ no MG também foi maior nos grupos LP em relação ao grupo CN. Não foi observada diferença entre os grupos no conteúdo de TNF- no MG. A expressão gênica de T-bet foi elevada nos grupos LP enquanto que a expressão de FOXP3 monstrou-se reduzida. A expressão de GATA3 foi reduzida no grupo 1LP e elevada no grupo 4LP. Em relação às alterações sistêmicas, foi observado um aumento de LPS, HSP70 e INF-γ nos grupos LP e redução de IL-4 nos mesmos grupos quando comparados ao CN. Nenhuma diferença foi observada entre os grupos em relação ao TGF-β. Baseados nos resultados do presente estudo, podemos inferir que tanto a imunidade inata como a imunidade adaptativa podem contribuir para a RI observada em ratos com LP. Dentre os possíveis mecanismos responsáveis por essas alterações destaca-se o recrutamento de macrófagos, ativação de vias inflamatórias no tecido muscular e uma modulação da expressão gênica de linfócitos no baço.
In the last few years, the relationship between oral infections and systemic disorders has been consolidated as an issue of great interest in the medical and dental scientific community. The periapical lesion (PL) is characterized as an oral inflammation and is associated with an increase in the levels of pro-inflammatory cytokines that possibly induce insulin resistance (IR). IR is defined as the inability of peripheral tissues to respond adequately to the physiological concentrations of this hormone, however, mechanisms that cause IR are not fully understood. Previous studies performed by our laboratory have found that PL promotes an increase in plasma TNF-α concentrations, impairment of insulin signal transduction, and reduction of plasma membrane GLUT4 content in skeletal muscle tissue, indicating a relationship between PL and IR. These findings highlight the need for further studies to verify the mechanisms involved in these changes. The present study was conducted in rats with PL and aimed: 1) to calculate the HOMA-IR index from the plasma glucose and insulin levels 2) to evaluate the total content of the inflammatory proteins (JNK, IKKα/β and TNF-α) in the gastrocnemius muscle (GM) and the JNK and IKKα/β phosphorylation status in the same tissue; 3) to verify the presence of infiltrated macrophages by F4/80 protein detection of in GM; 4) to quantify the expression of transcription factors involved in the lymphocytes differentiation into spleen (T-bet, GATA3 and FOXP3); 5) to analyze the plasma concentrations of LPS, HSP70, INF-γ, IL-4, TGF-β. For this purpose, 45 Wistar rats (2 months of age) were distributed in three groups: a) control rats without PL (CN); b) rats with PL induced in the upper right first molar (only one lesion) (1PL); c) rats with PL induced in the first and second upper and lower right molars (total of four lesions) (4PL). PL was induced using a surgical round bur 0.1 mm diameter. Statistical analysis was performed by analysis of variance (ANOVA) followed by the Tukey test. The adopted level of significance was 5% (alpha = 5%). The results demonstrated a reduction in insulin sensitivity and an increase in the macrophages content around the muscle fibers in the PL groups. The JNK and IKKα/β phosphorylation status in GM was also higher in the PL groups than in the CN group. No difference was observed among groups in TNF-α content in GM. The T-bet gene expression was elevated in the PL groups while the FOXP3 expression was reduced. GATA3 expression was reduced in the 1PL group and elevated in the 4PL group. In relation to the systemic alterations, an increase of LPS, HSP70 and INF-γ and a reduction of IL-4 were observed in the PL groups compared to the CN group. No difference was observed among groups in TGF- β concentrations. Based on the results of the present study, we can suggest that both innate and adaptive immunity may contribute to the IR observed in rats with PL. Among the possible mechanisms responsible for these changes are the macrophages recruitment, inflammatory pathways activation in muscle tissue and a modulation of the gene expression of lymphocytes in the spleen.
