リハビリテーション実施中の皮膚損傷の発生分析とその対策について

Abstract

急性期リハビリテーションにおけるリスク管理では，バイタルサインの変化やドレーン・チューブ類の管理が特に意識され ているが，実際の医療安全レポートにおける報告では皮膚損傷の頻度が高いと感じていた．方法は，医療安全レポートを後方 視的に調査し，皮膚損傷が生じたケースの傾向や特徴を分析した．その結果，軽微な処置が必要であった事象の約半数は皮膚 損傷の報告であり，転倒を伴わないものと伴うものがあった．転倒を伴わないものは高齢および脳血管疾患患者で多く，週の 後半での発生が目立った．また，経験年数の浅い職員での発生が多かった．転倒を伴うものは小児や血液疾患患者で多く，週 の前半での発生が目立った．対策として，結果を知識として共有することや，皮膚保護および安全な理学療法技術の習得支援 が必要と考えられた．

In risk management of the acute phase of rehabilitation, special attention is paid to changes in vital signs and drain and tube management, but a high frequency of skin injury is seen in incident reports. The purpose of this study was to analyze the incident reports of skin injury cases. About half of the incidents with minor treatment were skin injury with falls and without falls. Most cases without falls were in patients with cerebrovascular disease and elderly. The incidents occurred in the second half of the week and under inexperienced staffs. Most cases with falls involved children and patients with a blood disease, and occurred in the first half of the week.
