Resumen:
La asociación entre cáncer e inflamación en un órgano o tejido se encuentrasólidamente establecida. En efecto, se sabe que ciertos tumores, tienen una mayorprobabilidad de originarse en sitios de inflamación crónica y que procesosinflamatorios locales pueden acelerar el crecimiento de tumores preexistentes enanimales y seres humanos. Por otro lado, la relación entre cáncer e inflamaciónsistémica no ha sido particularmente estudiada. En esta tesis, demostramos quedurante el crecimiento de 4 tumores murinos, de diferente origen, estirpe histológicoe inmunogenicidad y de un tumor humano creciendo en ratones nude, se generaron 2 picos temporalmente separados de inflamación sistémica. El primer pico fuerelativamente pequeño y transitorio ya que fue detectado durante la primera semanaluego de la inoculación del tumor. Tal vez represente, en parte, una respuesta delorganismo al trauma mecánico de la inoculación de elementos extraños, ya que lainoculación de fibroblastos murinos normales también produjo una manifestación deinflamación sistémica, aunque menor, que la observada después de la inoculaciónde células tumorales. Por otro lado, el inicio del segundo pico de inflamaciónsistémica fue observado cuando el volumen tumoral fue mayor que 500 mm3 ycoincidente con el inicio del crecimiento exponencial de cada tumor. Este segundopico de inflamación sistémica fue significativamente más prominente que el primeroy su intensidad aumentó en relación directa con el incremento de la masa tumoral. Estos resultados sugieren que la inflamación sistémica podría ser un fenómenogeneral de la patología neoplásica y su importancia se puso de manifiesto en elhecho de que, en nuestra colección de tumores, se observó, sorpresivamente, unacorrelación directa entre la magnitud de la inflamación sistémica generada por cadatumor y su agresividad, correlación que no se observó con la capacidad de generarmetástasis. Esto indica, al menos en los tumores analizados por nosotros, que lacapacidad de generar una potente respuesta inflamatoria sistémica, produce másefectos deletéreos sobre el organismo que la propia capacidad metastásica,disminuyendo de este modo la sobrevida. En este sentido, se encontró graninfiltración de neutrófilos y alteraciones estructurales en diferentes órganos, que soncompatibles con disminución y aún pérdida de función, independiente de lacapacidad metastásica de cada tumor. La inflamación sistémica generada por eltumor exacerba el propio crecimiento tumoral estableciendo un circuito deretroalimentación positivo entre ambos, al menos en parte por la activación delreceptor TLR-4. Además, la disminución de las manifestaciones de inflamaciónsistémica mejoró los efectos anti-tumorales obtenidos con quimioterapia yradioterapia, aunque estos resultados fueron relativamente modestos. Esto se debea que el tumor cuando supera los 1500 mm3, comienza un crecimiento exponencial,generando un fenómeno de inflamación sistémica progresivo, a pesar de que secontinúan administrando las drogas antiinflamatorias, que hasta ese momentohabían sido bastante efectivas para limitar la naciente inflamación sistémica y pararetardar el crecimiento tumoral. En cambio, un excelente resultado terapéutico fuealcanzado cuando se realizó el tratamiento antiinflamatorio junto con la escisiónquirúrgica de tumores > 2000 mm3. En este caso, la eliminación quirúrgica de lafuente principal de inflamación sistémica y de posible refractariedad, permitió altratamiento antiinflamatorio tener muchas más chances de actuar, contra lainflamación sistémica residual, pero aún deletérea, que perdura al menos 2 semanasdespués de la extirpación del tumor. Si esta condición inflamatoria sistémica fuerauna característica distintiva de muchos canceres avanzados, un conocimiento máspreciso de los mecanismos por los cuales esta inflamación sistémica promueve elcrecimiento tumoral y produce múltiples daños en el organismo y la búsqueda desustancias antiinflamatorias más efectivas y con menores efectos adversos, seríanmuy importantes para complementar y mejorar las terapias actuales contra elcáncer.
Abstract:
The link between cancer and inflammation in an organ or tissue has firmly beenestablished on the basis that cancer tends to occur at sites of chronic inflammationand that local inflammatory processes can accelerate the growth of preexistingtumors in both animals and human beings. In contrast, the relationship betweencancer and systemic inflammation has been less studied. In this work, wedemonstrated that during the growth of 4 murine tumors of different origin,histological type and immunogenicity and 1 human tumor in nude mice, twotemporally separate peaks of systemic inflammation were detected. The first peakwas relatively small and transient detected during the first week after tumorinoculation. It probably represented, at least in part, the response of the organism tothe mechanical trauma of inoculation of foreign bodies, because the inoculation ofnormal fibroblasts produced a rather similar manifestation of systemic inflammationthan that observed after the inoculation of tumor cells. On the other hand, the onsetof the second peak was observed when the tumor was 500 mm3, coincidental withthe beginning of the exponential tumor growth. This second peak of systemicinflammation was significantly more prominent than the first one and its magnitudeincreased progressively in parallel with the increase of tumor mass. These resultssuggest that systemic inflammation could be a general phenomenon of neoplasticpathology and its importance became manifest in the fact, that in our tumorcollection, was observed, surprisely, a direct correlation between the magnitude ofsystemic inflammation generated by each tumor and its aggressiveness. On theother hand, in our tumor models, tumor aggressiveness did not correlate to thetumor´s ability to disseminate or metastasize, suggesting that the magnitude ofsystemic inflammation rather than the metastatic ability produce more deleteriouseffects in the organism, diminishing survival. In fact, was observed a large number of PMN and damage in tissues and organs, that are compatible with impair function,indepently of tumor´s metastatic ability. The systemic inflammation generated bytumor enhance tumor growth, establishing a positive feedback between both, at leastin part, by activation of TLR-4 receptor. In addition, the attenuation of themanifestations of systemic inflammation improves the anti-tumor effects obtainedwith radiotherapy and chemotherapy. However, this results were relatively modest. This is probably due to the fact that when tumor surpass 1500 mm3, begins anexponential growth, generating a systemic inflammation which magnitude increaseprogressively, even though the continue inoculation of the anti-inflammatory drugs. Instead, an excellent therapeutic result was obtained when the anti-inflammatorytreatment schedule was combined with surgery. In this case, the surgery eliminatedthe main source of systemic inflammation and in consequence an anti-inflammatorytreatment had a better chance to act against the residual, but still deleterious,systemic inflammation. If a systemic inflammatory condition were a hallmark of manyadvanced cancers, a more accurate knowledge of the underlying mechanisms bywhich systemic inflammation promotes tumor growth and produces multipledeleterious effects on the organism and the search for new more effectives and withlesser side effects anti-inflammatory factors will help to design new strategies tocomplement and to improve the current therapies against cancer.
