Cáncer de mama avanzado: análisis de ctdna y next generation sequencing (avenio-77) en la identificación de biomarcadores y su comportamiento clínico

Resumen
Objetivos: Describir la prevalencia de mutaciones por plataforma molecular
(Avenio-77) en pacientes con cáncer de mama avanzado (CMM) y su relación
con factores clínicos, biológicos y moleculares, para identificar pacientes con
indicación de terapia dirigida y correlacionar variables con la supervivencia
global.
Material y Métodos: Pacientes con edad media de 61 años (estadío IV-TNM)
entre 1-11-2019 y 31-12-2021 del Hospital Arnau de Vilanova (Valencia).
Variables de situación clínica inicial, histoquímicas, mutaciones genómicas y
de seguimiento terapéutico.
Resultados: 74 mujeres / 2 hombres; 46 sin tratamiento previo (60,5%). En 53
pacientes (69,7%) se detectaron mutaciones accionables (más prevalentes
PI3K, ERBB, ESR1). Correlación de mutaciones (p<0,05): fenotipo luminal -
mutaciones PI3K y ESR1; p53 - enfermedad visceral; ESR1 - pacientes en 1ª
línea de tratamiento; p53 - CEA alto; Ca 15.3 - ESR1 y p53; CTCs>5 -
presencia de mutaciones patológicas y ESR1. 34 pacientes progresaron a 1ª
línea de tratamiento (44,74%) y a 8 se les aplicó terapia dirigida (10,53%).
Conclusiones: El estudio de ctDNA en sangre permite identificar mutaciones
en CMM para valoración de terapia dirigida. La carga mutacional está
correlacionada con menor supervivencia. Las pacientes que se derivan a
terapia dirigida son principalmente en enfermedad luminal. La indicación de
terapia dirigida es escasa. La SG es menor en pacientes con CTCs>5,
enfermedad visceral y fenotipos HER2+CTMN. Objectives: To describe the prevalence of mutations by molecular platform

mutations (most prevalent PI3K, ERBB, ESR1) were detected in 53 patients
(69.7%). Correlation of mutations (p<0.05): luminal phenotype - PI3K and ESR1
mutations; p53 - visceral disease; ESR1 - patients in 1st line of treatment; p53 -
high CEA; Ca 15.3 - ESR1 and p53; CTCs>5 - presence of pathological mutations
and ESR1. 34 patients progressed to 1st line of treatment (44.74%) and 8 had
targeted therapy (10.53%)..
Conclusions: The study of ctDNA in blood allows identification of MSC mutations
for targeted therapy assessment. Mutational burden is correlated with lower
survival. Patients referred for targeted therapy are mainly in luminal disease.
Indication for targeted therapy is rare. OS is lower in patients with CTCs>5,
visceral disease and HER2+CTMN phenotypes..
