Avant-propos : La prise en charge des personnes nécessitant un traitement par hypocholestérolémiants peut être améliorée. De nombreux patients n’atteignent pas les valeurs fixées par les guidelines en termes de LDL-cholestérol. D’autres sont incapables ou refusent de prendre des statines, les molécules utilisées en premier choix. L’acide bempédoïque est un nouveau principe actif récemment commercialisé sur le marché belge, pouvant possiblement aider les praticiens et les patients face à ces deux problèmes. Méthode et objectifs principaux : Dans ce mémoire, l’intérêt est porté, dans un premier temps, sur la caractérisation pharmacologique de l’acide bempédoïque. Ses spécificités sont présentées et comparées à celles des principales molécules utilisées pour les mêmes indications dans le pays. Dans un deuxième temps, le travail est consacré à une analyse détaillée des études précédant l’autorisation de mise sur le marché et d’un essai clinique en cours évaluant son impact potentiel sur des critères forts. L’ensemble des informations récoltées permet de déterminer les intérêts de l’acide bempédoïque ainsi que ses inconvénients. Aussi, le mémoire discute quant à sa potentielle place au sein de l’arsenal thérapeutique des hypocholestérolémiants. Pour conclure, une prise de position argumentée est proposée sur base des données disponibles actuellement. Conclusion : L’acide bempédoïque est une molécule innovante présentant autant de points forts que de points faibles. En attendant des données approfondies concernant son efficacité sur des critères forts, il est nécessaire de choisir soigneusement les patients à traiter et d’être prudent quant aux effets secondaires du principe actif. Background : The management of people requiring lipid-lowering therapy can be improved. Many patients do not reach the guidelines value for LDL-cholesterol. Some others are uncapable or unwilling to take statins, the first molecules considered. Bempedoic acid is a new active ingredient recently marketed in Belgium, which may help practitioners et patients to deal with these two problems. Method and main objectives : In this thesis, the interest is first focused on the pharmacological characterisation of bempedoic acid. Its specificities are presented and compared with those of the main molecules used for the sames indications in the country. Secondly, the work is devoted to a detailed analysis of the studies preceding the marketing authorization and of a clinical trial in progress evaluating its potential impact on strong criteria. All the information gathered allows to determine the interests of bempedoic acid as well as its disadvantages. The thesis also discusses about the potential positioning among the cholesterol-lowering agents. In conclusion, a reasoned position statement is proposed based on currently available data. Conclusion : Bempedoic acid is an innovative molecule with as many strengths as weaknesses. More precise data concerning its efficacy on strong criteria are not available yet. Until then, it is necessary to carefully select the patients to be treated and to be cautious about the side effects of the active ingredient.
