Leiomyoma is a benign connectival tumor, that rarely arises on soft tissue. We present a case of a very big leiomyoma (about 20 cm in diameter) arising on soft tissue of perineum of 56 years old woman. After making a preoperative staging of the tumor and a preoperative planning for reconstruction, the tumor has been enucleated and the subgluteal and intergluteal folds have been reconstructed reshaping the skin expanded by the tumor. Histologically the tumor consisted of bundles, elongated spindle cells, no atypia and mitotic activity were seen. After 16 months no local recurrences have been noted clinically and on CT scans.
