소아암 환아의 형제자매들이 지각한 소외감과 사회적 지지

Abstract
소아암은 치명적인 특성과 더불어 치료기술의 발전으로 인해 만성적인 특성을 함께 가지게 되었다. 따라서 환아와 부모뿐 아니라 환아의 형제자매에게도 다양한 심리사회적 문제를 야기하게 된다.
본 연구는 소아암 환아의 형제자매가 느끼는 소외감과 일반적인 사회적 지지, 의료인 지지 지각의 정도를 파악하고 그 상관관계를 알아보며, 형제자매의 일반적 특성과 가족 특성에 따라 그 정도에 차이가 있는지 비교해보고자 수행하였다.
소외감은 Dean의 소외감 척도를 한영신이 수정한 도구를 사용하여 측정하였고, 일반적인 사회적 지지 지각은 Dubow와 Ulman의 사회적 지지 평가 척도를 김연희가 수정•번안한 척도를 이용하여 측정하였다. 의료인 지지 지각 척도는 Murray의 간호사와 소아암 환아 형제자매의 사회적 지지 질문지를 도구개발자의 허락을 얻은 후 번역, 역번역의 과정을 거쳐 사용하였다.
자료는 2011년 7월부터 2011년 12월까지 수집되었고, SPSS PASW statistics 18.0을 이용하여 분석하였다.

본 연구의 결과는 다음과 같다.
1. 대상자의 소외감은 42.24±12.72점으로, 하위영역은 규범상실, 사회적 고립, 무력감의 순으로 나타났다. 주관식 답변을 통해 형제자매의 심리사회적 취약성이 파악되었다.
2. 대상자가 지각하는 일반적인 사회적 지지 지각 점수는 92.30±13.33점으로 나타났고, 친구, 가족, 교사 지지의 순으로 나타났다.
3. 의료인 지지에 대해서는 빈도(62.20±21.76)는 낮게, 유용성(85.25±29.20)은 중간 정도로 지각하고 있었다. 빈도와 유용성 모두 평가적 지지가 가장 높았으며 다음으로 정서적, 정보적, 도구적 지지의 순이었다.
4. 대상자가 지각하는 소외감은 일반적인 사회적 지지와는 음적 상관관계를 보였으나(r=-.676, p<.001) 의료인 지지와는 상관관계를 보이지 않았다.
5. 대상자와 가족의 특성에 따른 소외감 지각의 차이는 성적이 나쁜 경우가 좋은 경우에 비해 소외감의 정도가 높았으며(F=6.047, p=.004), 아버지의 학력이 고졸인 경우 대졸 이상인 경우보다 소외감을 높게 느꼈다(t=2.589, p=.011).
6. 소아암 환아의 형제자매와 가족의 일반적 특성에 따른 의료인 지지 지각의 차이는 형제자매가 소아암에 대해 더 알고 싶다고 답한 경우 의료인 지지를 유용하게 느끼는 것으로 나타났다(F=3.138, p=.049).
이러한 결과를 보면, 소아암 환아의 형제자매의 소외감은 중간 이하의 수준으로 나타났으나 주관식 답변을 통해서는 소아암 환아의 형제 자매가 경험하는 심리사회적 어려움을 예상해 볼 수 있다. 또한 본 연구에서 의료인 지지와 소외감의 직접적인 상관관계는 나타나지 않았으나, 이는 의료인 지지에 대한 경험이 많지 않았기 때문일 수 있으며, 경험한 의료인 지지에 대해서는 어느 정도 유용하게 느끼는 것으로 결론지을 수 있다. 형제자매의 심리사회적 취약성이 소아암 환아의 진단과 치료라는 과정과 무관하지 않으므로, 그 중재를 위해서는 기존의 일반적인 사회적 지지와 더불어 의료인에 의한 지지의 제공에 대해서도 고려하여야 할 것이다.
Childhood cancer is one of the fatal diseases frequently accompanied by continuing family issues such as social and psychological problems for any of the patients siblings as well as their parents.
In this study, it was examined the siblings alienation, general social support, and perceived support from medical staffs, and the co-relations, and explored how they were different according to general characteristics of the siblings and those of family.
The alienation was measured by the revised model of the Dean Alienation Scale by Young-shin Han, and the general social support perception was quantified by the Yon-hee Kims version from Dubow and Ulmans Social Support Appraisers Scale. For medical staffs support perception scale, Murrays Nurse-Sibling Social Support Questionnaire was used after translation and back-translation, with the approval from the tool developer.
Data were collected from July 2011 through December 2011, and analyzed with the SPSS PASW statistics 18.0.

Following are the results:
1. The subjects alienation scored 42.24±12.72 points. The item was followed by normlessness, social isolation and powerlessness. When it was difficult for them to complete the multi-choice questions, short-answer questions were used to identify their social, psychological vulnerability.
2. The respondents general social support perception score was 92.30±13.33. And the highest value came from friends, followed by family and teachers.
3. For the medical staffs support, a low score (62.20±21.76) and medium level score (85.25±29.20) were reported for frequency and helpfulness, respectively. For both of these two items, the highest records were found in appraisal, emotional, informational, and instrumental in an orderly fashion.
4. The subjects perceived alienation showed negative co-relations with the general social support, with no relations with the medical staffs support.
5. With regard to the gap in alienation perception by the general status of the siblings and family, the alienation level was higher among the poor performance students, and the high school graduate fathers, compared to among the higher performance students, and fathers with college degrees or higher.
6. Regarding the gap in the medical staffs support perception according to the general characteristics of the siblings and family of the child with cancer, medical staffs support was regarded as useful when the respondents said they wanted to learn more about childhood cancer.
The results showed that alienation felt by the siblings of a child with cancer was below the medium level. However, their social, psychological vulnerability was able to be estimated through short-answer questions and preliminary research. Although the direct relations between medical staffs support and the siblings alienation were not found in this study, it was proved that they sensed the medium level of helpfulness for support from the medical staffs. Given the fact that their vulnerability is caused as a result of the diagnosis and treatment of childhood cancer, medical staffs support should be taken into consideration in addition to the general social support in order to help address the issue.
