Εισαγωγή: H λεπτίνη είναι ένας αγγειογενετικός παράγοντας που in vitro προάγει την μετανάστευση και ενεργοποίηση των ενδοθηλιακών κυττάρων και in vivo την αγγειογένεση, οδηγώντας στην δημιουργία νέων αγγείων.Αντικείμενο της μελέτης: Το αποτέλεσμα από την τοπική χορήγηση εξωγενούς λεπτίνης στην επιβίωση ολικού πάχους τυχαίων κρημνών σε ποντίκια.Υλικά και μέθοδοι: 90 Sprague-Dawley ποντίκια, βάρους 250-280 g χρησιμοποιήθηκαν στηνμελέτη. Σχεδιάστηκε, ένας ραχιαίος ολικού πάχους κρημνός (10cm×2cm), διατηρώντας τα αγγεία της ουράς στη βάση του, στο επίπεδο της λαγόνιου ακρολοφίας. Τα ποντίκια, χωρίστηκαν σε 10 ομάδες των 9 ζώων η καθεμία. Στην συνέχεια, χορηγήθηκαν ενδοδερμικά 1μg/ml VEGF (ομάδα E), 1 ml N/S 0.9% (ομάδα ελέγχου), 100 ng/ml λεπτίνης (ομάδα A), 250 ng/ml λεπτίνης (ομάδα B), 500 ng/ml λεπτίνης (ομάδα C) και 1000 ng/ml λεπτίνης (ομάδα D) σε 1mL n/s 0.9% για κάθε ένεση, στα άπω 2/3 του κρημνού σε 8 διαφορετικά σημεία. Ομάδα A1: 100 ng/ml λεπτίνης με 150 ng/ml αντιλεπτίνης, ομάδα B1: 250 ng/ml λεπτίνης με 375 ng/ml αντιλεπτίνης, ομάδα C1: 500 ng/ml λεπτίνης με 750 ng/ml αντιλεπτίνης, ομάδα D1: 1000 ng/ml λεπτίνης με 1500 ng/ml αντιλεπτίνης με σκοπό να μελετηθεί η αναστολή του παράγοντα της λεπτίνης. Η επιβίωση των κρημνών μελετήθηκε 3, 7 και 9 ημέρες μετά την επέμβαση. Σε εννιά ζώα έγινε έγχυση 1 mL N/S 0.9% και χρησιμοποιήθηκαν ως ομάδα ελέγχου.Αποτελέσματα: Ο βαθμός επιβίωσης των κρημνών στην ομάδα D (1000 ng/ml λεπτίνης) και στην ομάδα E (1μg/ml VEGF) ήταν αυξημένος συγκρινόμενος με εκείνον στις άλλες ομάδες. Μετά την θανάτωση των ζώων, το δέρμα των κρημνών αναλύθηκε αδρά με βάση την εμφάνιση, το χρώμα και την υφή του. Στην συνέχεια, τα δείγματα από τον τραυματικό ιστό από κάθε ομάδα μελετήθηκαν για τον καθορισμό της μικροαγγειακής τους πυκνότητας (MVD). Όλα τα ενδοθηλιακά κύτταρα καλλιεργήθηκαν με anti-rat CD 34 αντίσωμα, η οποία επιβεβαίωσε μεγαλύτερη αγγειογέννεση στην ομάδα D (1000 ng/ml λεπτίνης) συγκριτικά με όλες τις άλλες ομάδες.Ο μεγαλύτερος αριθμητικά σχηματισμός νέων αγγείων για διαφορετικές δόσεις λεπτίνης εμφανίστηκε στις μεγαλύτερες δόσεις. Όλα τα δείγματα των τραυμάτων από την ομάδα ελέγχου και τις ομάδες με χορήγηση λεπτίνης/αντιλεπτίνης δεν εμφάνισαν νέα αγγεία. Συμπέρασμα: Η τοπική χορήγηση ανασυνδυασμένης λεπτίνης ίσως βελτιώνει την επιβίωση ισχαιμικών δερματικών κρημνών και είναι άμεσα εξαρτημένη από τη δόση της.

The survival rate of the flaps was evaluated on day 3, 7 and 9 postop. Results: The survival rate of the flaps in the group D (1000 ng/ml leptin) and the group E (1mg/ml VEGF) increased significantly as compared with those of the other groups.Immunohistochemical staining showed that more granulation tissues and angiogenesis were observed in the (1000 ng/ml leptin) group than those in the VEGF, leptin/antileptin and the saline groups.After sacrifice of the animals the skin was determined grossly by its appearance, colour and texture. The wound tissue samples from each experimental group underwent evaluation of the level of microvessel density (MVD). All endothelial cells were stained with anti-rat CD 34 antibody. The biggest formation of new vessels for the different doses of the leptin appeared in the higher doses. All wounds from the control group and from injection of leptin and antileptin did not appear new vessels. Conclusion: Local application of recombitant leptin may efficiently improve the survival of ischemic skin flaps.
