Boezemfibrilleren is de meest voorkomende hartritmestoornis, die steeds meer voor zal komen door de vergrijzing van onze samenleving. Behandelingen voor boezemfibrilleren oefenen daardoor een toenemende druk uit op de zorg en de kosten van de zorg. Effectieve behandeling van boezemfibrilleren is daarom van groot belang. Echter, de mechanismes van boezemfibrilleren, en van recidieven van boezemfibrilleren na een behandeling worden nog niet volledig begrepen. Daarom beschrijf ik in dit proefschrift verschillende factoren die het succes van de behandeling kunnen bepalen. Daarvoor hebben we onderzoek gedaan naar patiƫnten die een zogenaamde thoracoscopische ablatie voor boezemfibrilleren krijgen. Dit stelde ons in staat om klinische factoren te onderzoeken die een relatie hebben met een succesvolle of niet succesvolle ablatie. Vervolgens hebben we deze karakteristieken gerelateerd aan structurele kenmerken van het hartspierweefsel, de elektrische geleiding van het hartspierweefsel, en de uitkomst na de behandeling. Deze combinatie van klinische, structurele en elektrofysiologische kenmerken geven belangrijke nieuwe inzichten in de mechanismes van boezemfibrilleren, en die van recidieven na een behandeling.
