Title: Resposta diferencial de células mononucleares do sangue periférico de indivíduos epiléticos e não epiléticos a estímulo inflamatório

Abstract: Epilepsia é considerada uma predisposição permanente para geração de crises convulsivas, com a presença de disfunção cognitiva e emocional do indivíduo, sendo uma das desordens neurológicas mais comuns em todo o mundo, afetando 1% da população mundial. O papel da inflamação na epilepsia humana foi primeiramente investigado através do tratamento com esteroides e outros anti-inflamatórios, quemostraram uma ação anticonvulsivante em alguns casos de epilepsia resistente ao tratamento convencional. Evidências sugerem a ativação da imunidade inata e adaptativa na epilepsia humana, sugerindo também que a inflamação pode contribuir para geração e recorrência de crises e dano neuronal, por outro lado, as crises epiléticas recorrentespodem perpetuar a resposta inflamatória, o que leva a uma inflamação crônica. Apesar disso, ainda não é claro se a produção de mediadores inflamatórios por células imune de pacientes portadores de epilepsia diferem das de indivíduos saudáveis. Desta forma, o presente estudo pretendeu testar a hipótese de que existe uma desregulação em vias desinalização de células mononucleares periféricas de pacientes com ELT, o que contribui para um padrão de resposta diferencial em relação aos indivíduos controles frente a um estímulo inflamatório. Métodos: Os experimentos foram realizados em indivíduos com Epilepsia do Lobo Temporal com e em indivíduos saudáveis, entre 18 e 65 anos, queaceitaram participar da pesquisa, assinando o Termo de Consentimento Livre e esclarecido (TCLE). Os indivíduos que aceitaram participar do estudo e que preenchiam os critérios estabelecidos pela pesquisa foram submetidos a uma breve anamnese e coleta de dados sócio-demográficos, aferição de peso e altura, aplicação do questionário para avaliação de sintomas ansiosos e depressivos (HAD) e coleta do material biológico (sangue periférico). O sangue periférico foi recolhido em tubos a vácuo contendo heparina sódica e foi realizado o protocolo de Obtenção de Células Mononucleares do SanguePeriférico (CMSP). Após a contagem das células, foi realizado o plaqueamento e as células foram submetidas à cultura na presença ou ausência dos bloqueadores das enzimas PI3K, PI3Kg, mTOR e GSK3 por uma hora, em estufa de CO2 (37ºC). Após essa breve incubação, adicionou-se PHA aos poços estimulados ou RPMI aos poços semestímulos. As células foram então incubadas por 24 horas em estufa de CO2. Após a incubação, separou-se o sobrenadante das culturas e estes foram armazenados em freezer -20ºC até o momento das análises. As dosagens de citocinas e quimiocinas do sobrenadante das culturas e plasma dos indivíduos com ELT e controles foram realizadaspelo método de CBA, conforme instruções do fabricante (BD Bioscience, San Diego, CA, USA). Utilizaram-se os kits para quantificação de proteínas inflamatórias (Human Inflammation - IL-1, IL-6, IL-10, TNF e IL-12p70). A análise estatística foi realizada utilizando o programa estatístico Prisma 5.0 (GraphPad, CA, USA) e os dados foramapresentados como média ± erro-padrão da média (E.P.M.). O nível de significância foi estabelecido em p<0,05. Resultados: A inibição de todas as isoformas de PI3K ou a inibição seletiva de PI3Kg, assim como a inibição de mTOR aumentou os níveis de, IL-6 e IL-8 apenas em células de controles não estimuladas. Por outro lado, os inibidores nãoalteraram significativamente os níveis das células estimuladas com PHA. A inibição de todas as isoformas de PI3K ou a inibição seletiva de PI3Kg, assim como a inibição de mTOR e GSK3 resultou em uma diminuição nos níveis de TNF e IL-10 em células de pacientes com ELT estimuladas com PHA, com exceção da inibição de GSK3 para IL-10.Demonstramos ainda que PHA aumentou os níveis de IL-12p70 somente em células de pacientes e a inibição de PI3K e mTOR exacerbou o efeito de PHA somente nas células dos pacientes. A inibição seletiva da isoforma da PI3K não alterou os níveis de IL-12p70.Conclusão: No presente estudo demonstramos que a produção de citocinas por células imunes de pacientes com ELT é controlada de maneira diferenciada das células de controles saudáveis. Essa regulação diferencial pode ser dependente da atividade dediferentes moléculas intracelulares, tais como a via PI3K, mTOR e GSK-3. Esta diferente resposta a estímulos inflamatórios e uma alteração nas vias que controlam a produção de mediadores inflamatórios pode contribuir para a fisiopatogênese das epilepsias.
Abstract: Introduction: Epilepsy is considered a strong predisposition for the generation of seizures, in the presence of cognitive and emotional dysfunction and is one of the most common neurological disorders worldwide, affecting 1% of the population. The role of inflammation in human epilepsy was first investigated by treatment with steroids and other anti-inflammatories, which showed an anticonvulsant effect in some epilepsy cases resistant to conventional treatment. Evidence suggests the activation of innate and adaptive immunity in human epilepsy, also suggesting that inflammation might contributeto the generation and recurrence of seizures and neuronal damage, on the other hand, recurring seizures may perpetuate the inflammatory response, leading to inflammation chronic. However, it is not clear whether the production of the inflammatory mediators by immune cells of patients with epilepsy and healthy subjects differ. Thus, in the presenthypothesis, we aimed to test the hypothesis that there is a deregulation in the intracelular signaling pathways in the peripheral mononuclear cells of patients with epilepsy, which contributes to a differential pattern in comparison with healthy subjects in response to an inflammatory stimulus. Methods: Experiments were performed in patients with Temporal Lobe Epilepsy and in healthy subjects, between 18 and 65 years old, who agreed to participate by signing the Informed Consent (IC). Individuals who agreed to participate in the study and who met the criteria established by the research underwent a brief medical history and collecting sociodemographic data, weight measurement and height, thequestionnaire application for assessment of anxiety and depressive symptoms (HAD) and collection the biological material (peripheral blood). Peripheral blood was collected in vacuum tubes containing sodium heparin and was performed Obtaining Mononuclear CellsPeripheral Blood protocol (PBMC). After counting cells, plating was performed and the cells were subjected to culture in the presence or absence of blocker of the enzymes PI3K, PI3Kg, mTOR and GSK3 for one hour in the CO2 incubator (37°C). After this brief incubation, PHA was added to stimulated wells or RPMI to wells without stimuli. The cellswere then incubated for 24 hours in the CO2 incubator. After incubation, the supernatant was separated from these cultures and were stored in a freezer at -20°C until the time of analysis. Dosages of cytokines and chemokines in the culture supernatant and plasma of patients with ELT and controls were performed by CBA method, according to manufacturer's instructions (BD Bioscience, San Diego, CA, USA). Kits were used for quantification of inflammatory proteins (Human Inflammation - IL-1, IL-6, IL-10, TNF and I L-12p70). Statistical analysis was performed using the statistical software Prism 5.0 (GraphPad, CA, USA) and data were presented as mean ± standard error of the mean(SEM). The level of significance was set at p <0.05. Results: Inhibition of all isoforms of PI3K or the selective inhibition PI3Kg, as mTOR inhibition increased levels of IL-6 and IL-8 only in unstimulated cell controls. Furthermore, the inhibitors do not significantly alter the levels of cells stimulated with PHA. Inhibition of all isoforms of PI3K or the selectiveinhibition PI3Kg, as well as inhibition of GSK3 and mTOR, resulted in a decrease in the levels of TNF and IL-10 in epileptic patients cells, stimulated with PHA, with the exception of inhibition for GSK3 in IL-10. PHA also demonstrated an increased IL-12p70 levels only in cells of epileptic patients and inhibition of PI3K and mTOR exacerbated the PHA effect only in the cells of patients. The selective inhibition of PI3K isoform did not affect the IL-12p70 levels. Conclusion: In this study was demonstrated that cytokine production by immune cells from patients with ELT is controlled differently in cells from healthy controls. This differential regulation may be dependent on the activity of different intracellular molecules such as PI3K, mTOR and GSK-3. This different response to inflammatory stimuli and the alteration in the intracellular signaling pathways could contribute to the physiopathogenesis of epilepsies.
