Title: A doula comunitária: uma experiência reinventada

Abstract: Este estudo tem como objetivo analisar, sob a perspectiva das próprias doulas, as características pessoais das mulheres-doulas e suas práticas no cuidado às parturientes usuárias do Sistema Único de Saúde no Hospital Sofia Feldman. Caracteriza-se como um estudo descritivo-exploratório, valendo-se da metodologia qualitativa, sustentado no materialismo dialético. O cenário deste estudo foi a Fundação de Assistência Integral à Saúde/Hospital Sofia Feldman, em Belo Horizonte, Minas Gerais. Trata-se de uma instituição filantrópica que presta assistência à mulher e ao recém-nascido, usuários do Sistema Único de Saúde e que adota, como missão, a humanização da assistência. As participantes da pesquisa foram nove doulas comunitárias que atuam na Instituição há mais de dois anos. A coleta de dados ocorreu no período de 09/05 a 28/05/08, por meio da história de vida e de observação participante. Os relatos obtidos, nas histórias de vida, foram submetidos à análise de discurso segundo a orientação de Foucault (2006; 2008). A partir da análise dos discursos, constituíram-se sete formações discursivas: inserção no projeto; experiências próprias de parto; ser doula; relacionamento com a equipe institucional; satisfação; reconhecimento e o hospital-local de atuação. Os resultados revelaram que essas doulas são mulheres que atuam na vida pessoal e buscam construir uma inserção social que lhes permite contribuir de forma significativa para a vida de mulheres parturientes. Essas mulheres-doulas mostraram que a força que possuem e que as mantém é proveniente do cuidado diferenciado prestado por elas e damaneira como estabelecem as diversas relações que envolvem aassistência a uma parturiente, dentro de um hospital. Essas mulheres-doulas revelaram que a prática lhes ensinou a importância de serem prudentes e, às vezes silenciosas, como razão de continuar. Elasevidenciaram, ainda, o significado de outra dimensão do cuidado, possível a partir do verdadeiro encontro entre pessoas, um cuidado personalizado e humanizado. A nosso ver, a revelação mais rica foi que a força instituinte das doulas situa-se na diferença e no potencial que possuem para inspirar a mudança da mentalidade dos diversos atores que atuam na prática de assistência ao parto e nascimento
Abstract: This study aims at analyzing the personal characteristics of doula -women, according to their own point of view, and their practice in taking care of the parturient, who are users of the Unified Health System (SUS), from Sofia Feldman Hospital. This descriptive-explorative study was based on a qualitative methodology, sustained by dialect materialism. The research was developed at the Health Integral Assistance Foundation from Sofia Feldman Hospital in Belo Horizonte Minas Gerais, which is a philanthropic institution responsible for assisting the woman and the newborn who are SUS users and has as a mission the humanization of health care. A number of nine doulas, who work at the Foundation for more than two years, was studied. The data collection occurred from May 5th to 29th, 2008 taking into consideration the participants history of life and observation. The reports based on the history of life were evaluated according Foucaults (2006, 2008) orientation on discourse analysis. Considering discourse analysis, a total of seven discursive formations were constituted, consisted of: i) insertion on the project; ii) own experience during the parturition; iii) being a doula; iv) relationship with the institute staff; v) satisfaction; vi) recognition and vii) hospital (place of acting). The results have shown doulas are women who act in personal life and try to develop a social insertion that allows them to make an important contribution for the life of the parturient. Also, women-doulas have shown the strength they have, and which keeps them, comes from the differentiated care they give and the manner how they establish the several relations involving care to a parturient inside a hospital. In addition, they have also proved the meaning of the other dimension of taking care which was possible due to the true contact among people, a personalized and humanized care. The most important revelation in this study was that the doulas strength is situated on the difference and on the potential they have to inspire the change on the mentality of the various actors that helps during the parturition and the childs birth.
