Resumo: Este estudo pretendeu identificar e conhecer o caminho percorrido pelos bebês recém-nascidos prematuros extremos, entre a alta hospitalar e o serviço de reabilitação física no município de Campinas, na percepção das mães desses bebês. A prematuridade é considerada como um dos grandes problemas de saúde pública, contribuindo com elevados números para a morbimortalidade infantil e para a invalidez. Os recém-nascidos prematuros extremos que sobrevivem ao período neonatal, apresentam um risco elevado de intercorrências no neurodesenvolvimento a longo prazo. No município de Campinas existem alguns ambulatórios especializados no atendimento e seguimento desses bebês prematuros pós-alta hospitalar, dentre eles o Centro de Referência em Reabilitação Física (CRR). Sabe-se que neste serviço existem vagas remanescentes de acolhimento da reabilitação infantil que poderiam ser absorvidas por qualquer Centro de Saúde do município, para o encaminhamento dessa clientela. Trata-se de uma pesquisa de caráter qualitativo onde o recurso metodológico utilizado para o levantamento de dados primários acorreu através da realização de entrevistas semiestruturadas com as mães de bebês prematuros extremos, usuárias do CRR, onde foram questionadas sobre sua gestação e quais os encaminhamentos recebeu na alta hospitalar. O tamanho da amostra foi definido pelo método de amostragem por saturação, caracterizando 10 entrevistas. Foi adotada para o tratamento dos dados, a análise de conteúdo, o que resultou em três principais eixos-temas relacionados às demandas dessas mães em relação ao processo de acompanhamento e reabilitação de seus bebês. Na sequência, as discussões e as considerações foram respaldadas pelos conceitos referenciais da Saúde Coletiva. Foram encontrados: (1) encaminhamentos recebidos abrangendo o resumo de alta hospitalar, o processo de alta hospitalar referenciada e a interlocução entre a atenção hospitalar e a atenção básica; (2) percurso de acesso com apontamentos sobre o desconhecimento do fluxo de acesso estabelecido na rede e a dificuldade de acesso; e (3) percepção do modelo de atenção oferecido no serviço com a valorização da oferta terapêutica e do tratamento interdisciplinar, a ruptura do vínculo com os profissionais, a oferta de atendimentos reduzida e aspectos relacionados à acessibilidade prejudicados, especialmente pela oferta de atendimentos, distância geográfica (localização) e fragilidade nas políticas de transporte

Abstract: This study aimed to identify and know the path taken by the infants of premature newborns, between the hospital discharge and the physical rehabilitation service in the city of Campinas, in the perception of the mothers of these infants. Prematurity is considered as one of the major public health problems, contributing with high numbers for infant morbidity and mortality and for disability. Preterm infants surviving the neonatal period are at high risk for long-term neurodevelopmental complications. In the city of Campinas, there are some outpatient clinics specialized in the care and follow-up of these preterm infants, including the Reference Center for Physical Rehabilitation (CRR). It is known that in this service there are remaining vacancies of child rehabilitation that could be absorbed by any Health Center in the municipality, for the referral of this clientele. It is a qualitative research where the methodological resource used for the primary data collection was carried out through semi-structured interviews with the mothers of extremely premature infants, users of CRR, where they were questioned about their gestation and which referrals received at hospital discharge. The sample size was defined by the saturation sampling method, characterizing 10 interviews. It was adopted for the data treatment, content analysis, which resulted in three main axes-themes related to the demands of these mothers in relation to the process of monitoring and rehabilitation of their babies. In the sequence, the discussions and the considerations were supported by the referential concepts of Collective Health. The following were found: (1) referrals received covering the summary of hospital discharge, the hospital referral process and the interlocution between hospital care and basic care; (2) access route with notes about the lack of knowledge of the access flow established in the network and the difficulty of access; and (3) perception of the care model offered in the service with the valorization of the therapeutic offer and the interdisciplinary treatment, the rupture of the bond with the professionals, the reduced attendance offer and aspects related to accessibility impaired, especially by the offer of services, distance (location) and fragility in transport policies
