Eficácia e segurança de antifúngicos sistêmicos em infecções fúngicas invasivas : revisão sistemática em meta-análise

Resumo: Infecções fúngicas invasivas (IFIs) possuem índices elevados de morbidade e mortalidade, especialmente em pacientes imunocomprometidos. Nos últimos 30 anos houve um aumento destas infecções em seres humanos, acompanhado por um crescente número de antifúngicos. Em vista disso, o objetivo deste estudo foi realizar uma avaliação e comparação de evidências primárias e secundárias sobre eficácia e segurança de antifúngicos sistêmicos no manejo de IFIs. Para tal, foi realizada uma overview de ensaios clínicos randomizados (ECRs), na qual obteve-se 990 registros através da busca em base de dados e da busca manual. Dentre estes, 34 estudos foram incluídos por contemplarem os critérios de inclusão, obtendo uma média de 2355 pacientes avaliados. A qualidade destes estudos variou de baixa a alta. Além disso, foi realizada uma revisão sistemática e meta-análise de ECRs duplo-cegos que compararam os antifúngicos de interesse entre si ou com placebo. Foram encontrados 10891 registros através da busca em base de dados e da busca manual, dentre os quais 67 foram incluídos por contemplarem os critérios de inclusão, fornecendo um número de 16336 pacientes avaliados. Um maior número de homens foi encontrado (59,1%) e a idade média ponderada dos participantes foi de 42,4 ± 18,1 anos. Na profilaxia de IFI, houve superioridade de anfotericina B lipossomal e triazólicos de primeira geração frente ao placebo, não sendo detectada diferença para os desfechos de segurança. Nas terapias empírica e definida, as equinocandinas apresentaram-se mais seguras, porém sem diferença estatisticamente significativa para o desfecho de eficácia em relação aos poliênicos. Devido à ausência de comparações entre os fármacos analisados para a eficácia na terapia profilática, foi realizada uma MTC (mixed treatment comparison). Combinando dados referentes a comparações diretas e indiretas entre pares de tratamentos, foi encontrada diferença de eficácia estatisticamente significativa entre anfotericina B lipossomal e triazólicos de segunda geração frente ao placebo e ao cetoconazol. Conforme o ranqueamento de eficácia, a melhor e pior terapia são voriconazol e cetoconazol, respectivamente. Os resultados apresentados neste trabalho podem contribuir na elaboração de protocolos clínicos na medida em que aumentam a qualidade da evidência disponível na seleção da terapia mais eficaz e segura no manejo de IFIs. No entanto, sabe-se que existe uma diferença de custo entre as estratégias terapêuticas avaliadas, e que, geralmente, este custo é elevado para os antifúngicos mais novos e que, por conseguinte, são recomendados nas diretrizes existentes. Em vista disso, sugere-se a incorporação de análises farmacoeconômicas ao estudo, a fim de definir a melhor relação custo-efetividade. Abstract: Invasive fungal infections (IFIs) have high morbidity and mortality, especially among immunocompromised patients. In the last 30 years, there has been an increase of this type of infection in human beings, but the number of antifungal drugs has increased as well. For this reason, the aim of this study was to evaluate and compare primary and secondary evidences about efficacy and safety of systemic antifungals in the management of IFIs. It was realized an overview of systematic reviews of randomized clinical trials (RCTs). At the beginning, it was obtained 990 records through databases and manual searches. Among these, 34 articles were included because they beheld the inclusion criteria. An average, 2355 patients were evaluated. The quality of these studies ranged from low to high. Furthermore, it was performed a systematic review and a meta-analysis of double-blinded RCTs which compared antifungals head to head or against placebo. 10891 records were founded through data bases and manual searches. Among these, 67 articles were included because they beheld the inclusion criteria, resulting in the number of 16336 patients evaluated. A bigger number of men was founded (59.1%) and the weighted age of the participants was 42.4 ± 18.1 years. In the prophylaxis of IFIs, the liposomal amphotericin B and the first generation triazoles were superior to placebo, but no difference was found in the safety outcome. In the empiric and defined therapy, the echinocandins were safer, but without statistic difference for the outcome efficacy when compared with polyenics. Due to the absence of comparability between drugs in the prophylactic regimen, it was performed a mixed treatment comparison (MTC). Combining data from direct and indirect comparisons between treatment pairs, it was founded statistically significant superiority for liposomal amphotericin B and second generation triazoles against placebo and ketoconazole. According to the efficacy ranking, the best and the worst therapy are, respectively, voriconazole and ketoconazole. The results of this study may contribute to the development of clinical protocols, since they enhance the quality of the evidence in the choice of the most effective and secure antifungal. Otherwise, it is known that a difference in the cost between the evaluated therapies exists, and usually this cost is higher for the newer antifungals, and these newer antifungals tend to be recommended in the existing guidelines more often. For this reason, it is suggest an incorporation of an economic analysis to this study, so it will be possible to define the best cost-effectiveness.
