Variabilidade nos genes da insulina e pax4 em crianças com diabetes mellitus tipo 1

Resumo: Diabetes mellitus tipo 1 (DM1) é uma doença grave que afeta um grande número de pacientes jovens. É caracterizada pela incapacidade do organismo para produzir insulina devido a destruição auto-imune das células beta pancreáticas. Corresponde cerca de 10% do número de diabéticos e tem aumentado cerca de 3% a cada ano, ocorrendo principalmente em crianças. O objetivo do trabalho foi avaliar a associação de polimorfismos de nucleotídeo único dos genes da Insulina (rs3842752) e PAX4 (Paired Box4) (rs 2233575 e rs712701) com o DM1 em um estudo de caso-controle e a correlação destas variações com os marcadores bioquímicos de perfil glicêmico (glicose, Hemoglobina glicada , 1,5-AG), perfil lipídico (colesterol total, colesterol-HDL, colesterol-LDL, triglicerídeos), função renal (ureia, creatinina) e, dados clínicos das crianças estudadas (idade, peso, IMC, Z-score, histórico familiar de DM, histórico de cetoacidose diabética). Foram estudadas 316 crianças Euro-brasileiras, não relacionadas, sendo 148 crianças com DM1 e 168 crianças saudáveis (grupo controle). As genotipagens dos polimorfismos selecionados para o estudo foram realizadas por PCR-RFLP (rs3842752 e rs2233575) e por TaqMan® (rs712701). O projeto teve aprovação do comitê de ética da UFPR (CAAE: 24676613.6.0000.0102). Crianças com DM1 apresentaram Z-score significativamente menor (P<0,05) quando comparado ao grupo controle. As concentrações séricas de glicemia em jejum, HbA1c, colesterol total, HDL, LDL, ureia e creatinina foram significativamente maiores no grupo com DM1. Entretanto, o grupo controle apresentou concentrações séricas significativamente maiores quanto a 1,5-AG, triglicérides, ácido úrico e proteínas totais. Os polimorfismos em estudo estão no equilíbrio de Hardy-Weinberg. Para o grupo saudável os alelos de menor frequência apresentaram respectivamente 15,8% (95%IC 12-20) para o rs3842752, 26,5% (95%IC 22-31), para o rs712701. Já o rs2233575 não se detectou o alelo de menor frequência. O polimorfismo rs712701 do gene PAX4 não foi associados ao DM1. O polimorfismo rs3842752 foi associado ao aumento de glicose e creatinina no grupo DM1. As frequências genotípicas e alélicas do polimorfismo rs3842752 do gene da insulina apresentaram diferenças significativas (P<0,05) entre os grupos estudados, sendo associado ao DM1. Portadores do alelo T possuem risco reduzido para desenvolver DM1 em cerca de um terço com OR: 0,31 (95%IC 0,17-0,55) e os portadores do alelo C possuem risco aumentado em cerca de 3 vezes de desenvolver DM1 com OR: 3,28 (95%IC 1,83-5,87). Abstract: Type 1 diabetes mellitus (T1DM) is a severe disease affecting a large number of young patients. It is characterized by the body's inability to produce insulin due to the autoimmune destruction of the beta cells in the pancreas. It corresponds to about 10% of the number of diabetics and has increased about 3% per year, occurring mostly in children. The aim of the study was to evaluate the association of single nucleotide polymorphisms of Insulin gene (rs3842752) and PAX4 (Paired Box4) (rs 2233575, and rs712701) with T1D in a case-control study, and the correlation of these changes with biochemical markers of glycemic profile (glucose, HbA1c, 1,5-AG), lipid profile (total cholesterol, HDL-cholesterol, LDL-cholesterol, triglycerides), renal function (urea, creatinine), total protein, albumin, uric acid, and clinical data of the children (age, weight, BMI, Z-score, DM family history, diabetic ketoacidosis history). Have been studied 316 Euro-Brazilian children, and the subjects were classified as T1D patients (T1DM, n= 148), and healthy children (control, n= 168). The genotyping of the polymorphisms was performed by RFLP-PCR (rs3842752, and rs2233575), and by the method TaqMan® (rs712701). The project was approved by the Ethics Committee UFPR (CAAE: 24676613.6.0000.0102). Children with T1DM presented Z-score lower compared to the control group. Serum concentrations of fasting glucose, HbA1c, total cholesterol, HDL, LDL, urea and creatine were significantly higher in the group with T1D. However, the control group had significantly higher serum concentrations as 1,5-AG, triglycerides, uric acid and total protein. The studied variants were in the Hardy- Weinberg equilibrium. For the control group, the frequency of minor alleles was 15.8% (95% CI 12-20) for rs3842752, 26.5% (95% CI 22-31) for rs712701 respectively, while for the rs2233575 was not observed allele lower frequency. The rs712701 polymorphism, in the PAX4 gene was not associated with T1D. The rs3842752 polymorphism was associated with increased glucose and creatinine in T1DM group. The genotype and allelic frequencies of the rs3842752 polymorphism of the insulin gene, differed significantly between the study groups (P<0.05), being associated with T1D. Carriers of the T allele of rs3842752 have a reduced risk to develop T1D in about one third OR: 0.31 (95% CI 0.17 to .55). However, carriers of the C allele have an increased risk by about 3 times to develop T1DM, OR: 3.28 (95% CI 1.83 to 5.87).
