Estudo do perfil cognitivo e da qualidade de vida em adultos com esclerose múltipla

Resumo: A Esclerose múltipla (EM) é considerada uma doença crônica que acomete mais frequentemente jovens adultos. Estima-se que 2,3 milhões de pessoas no mundo são afetadas pela doença. Pessoas com EM costumam apresentar alterações cognitivas e na sua qualidade de vida. A presente pesquisa visa identificar a ocorrência de tais alterações e estudar suas possíveis relações. Foram realizados três estudos. O primeiro apresentou como objetivo discutir os instrumentos de avaliação neuropsicológica utilizados atualmente com pessoas com diagnóstico de EM. Para tanto, foi realizada uma revisão sistemática da literatura cientifica. Este estudo mostrou a importância da investigação das funções cognitivas em pessoas com diagnóstico de EM. As baterias breves mais utilizadas evidenciadas na revisão sistemática são Brief International Cognitive Assessment of Multiple Sclerosis (BICAMS), Multiple Sclerosis Functional Composite Measure (MSFC) e Brief Repeatable Battery of Neuropsychological Test (BNR-N). Os testes mais utilizados na avaliação neuropsicológica de paciente com EM são PASAT e SDMT, que enfocam a avaliação da velocidade de processamento. O segundo estudo buscou investigar as características e peculiaridades cognitivas em pessoas com EM. Participaram deste estudo 41 pessoas com diagnóstico de EM e que foram entrevistadas e realizaram uma avaliação neuropsicológica composta pelos seguintes instrumentos: Teste de Figura Complexa de Rey-Osterrieth, Trail Making Test, Teste de aprendizagem Auditivo-Verbal de Rey, Bateria Psicológica para avaliação da Atenção, Teste dos cinco Dígitos, Teste de fluência verbal fonético e semântico e Symbol Digit Modalities Test. Os resultados evidenciaram que 48,7% da amostra apresentaram prejuízo cognitivo. Observou-se uma variabilidade em termos de funções prejudicadas, as que mais encontraram-se alteradas foram as funções de memória verbal imediata, flexibilidade cognitiva, fluência verbal fonética e capacidade atencional. Os prejuízos das funções executivas de flexibilidade cognitiva e controle inibitório chamaram atenção devido à grande variabilidade nos testes. Foi verificado na análise de correlação que quanto maior a idade e tempo de diagnóstico, pior o desempenho em atividade que exigem memória de curto prazo verbal. Além disso, não houve uma associação entre queixas auto referidas e o desempenho cognitivo, mostrando que o funcionamento cognitivo integro não esteve relacionado com a queixa subjetiva das pessoas da amostra. O terceiro estudo objetivou elucidar mais profundamente o impacto da (in) capacidade cognitiva, do sintoma de fadiga e das alterações psíquicas na qualidade de vida de pessoas com EM. O estudo foi efetivado com 41 pessoas com EM, utilizando os mesmos testes neuropsicológicos do estudo 2 comparando seus resultados com os das escalas como: Escala de Determinação Funcional da Qualidade de Vida em pacientes com EM; Escala Modificada de Impacto da qualidade de Vida; Escala Hospitalar de Ansiedade e Depressão; e Expanded Disability Status Scale (EDSS). Diante da análise dos dados coletados, pode-se constatar que a EM interfere na qualidade de vida em diferentes domínios e o estudo evidenciou a importância dos cuidados com os sintomas subjetivos como a fadiga e as implicações psicológicas desde o início do diagnóstico de EM para a manutenção da qualidade de vida. Abstract: Multiple sclerosis (MS) is considered a chronic disease that affects young adults more often. It is estimated that 2.3 million people worldwide are affected by the disease. People with MS usually have cognitive changes and their quality of life. The present research aims to identify the occurrence of such changes and to study their possible relationships. Three studies were carried out. The first one aimed to discuss the instruments of neuropsychological evaluation currently used with people diagnosed with MS. For this, a systematic review of the scientific literature was carried out. This study showed the importance of investigating cognitive functions in people diagnosed with MS. The most frequently used brief batteries in the systematic review are Brief International Cognitive Assessment of Multiple Sclerosis (BICAMS), Multiple Sclerosis Functional Composite Measure (MSFC) and Brief Repeatable Battery of Neuropsychological Test (BNR-N). The most used tests in the neuropsychological evaluation of MS patients are PASAT and SDMT, which focus on the evaluation of the speed of processing. The second study sought to investigate cognitive characteristics and peculiarities in people with MS. Thirty-one people with a diagnosis of MS were interviewed and carried out a neuropsychological evaluation composed of the following instruments: Rey-Osterrieth Complex Figure Test, Trail Making Test, Rey Auditory- Verbal Learning Test, Psychological Battery for evaluation of Attention, Five Digits Test, Test of verbal phonetic and semantic fluency and Symbol Digit Modalities Test. The results showed that 48.7% of the sample presented cognitive impairment. There was a variability in terms of impaired functions; the ones that were most affected were the functions of immediate verbal memory, cognitive flexibility, phonetic verbal fluency, and attentional capacity. Impairment of executive functions of cognitive flexibility and inhibitory control drew attention because of the great variability in the tests. It was verified in the correlation analysis that the greater the age and time of diagnosis, the worse the performance in activity that require short-term verbal memory. In addition, there was no association between self-reported complaints and cognitive performance, showing that full cognitive functioning was not related to the subjective complaint of the people in the sample. The third study aimed to elucidate more deeply the impact of cognitive (in) ability, fatigue symptom and psychic changes on the quality of life of people with MS. The study was carried out with 41 people with MS, using the same neuropsychological tests of study 2 comparing their results with those of the scales as: Functional Determination Scale of Quality of Life in MS patients; Modified Impact of Quality of Life Scale; Hospital Anxiety and Depression Scale; and Expanded Disability Status Scale (EDSS). In the analysis of the data collected, it can be seen that MS interferes in the quality of life in different domains and the study evidenced the importance of the care with subjective symptoms such as fatigue and the psychological implications from the beginning of the diagnosis of MS to the maintenance of quality of life.
